Office Hours

Office hours are Monday, Wednesday, Thursday and Friday 9AM-5PM. Weekly schedules vary based on university, teaching and sleep center commitments.

Emergencies

If you are in an emergency situation that cannot wait until normal business ours, you can notify our service after 5PM and your call will be returned.

Initial Consultation

Please make sure you bring your insurance card(s) in addition to a copy of you sleep study. Please plan on arriving 15minutes early in order to fill out necessary paperwork, or save time by requesting we mail or fax you a copy to fill out prior to your visit.

Referral Policy

If your insurance plan requires a referral, it is your responsibility to call your primary care physician prior to your appointment and obtain one. For your convenience you can call and request they fax it to our office 631-581-2777. A referral maybe essential for claim processing.

Prescription Refills

Should you require premedication for dental treatment please call the office at least 24 hours prior to your visit so we may phone it in for you. All other refills will be evaluated on a case by case basis.

Confidentiality: Medical/Dental Records

All patient records are confidential; this office maintains compliance with HIPPA regulations regarding confidentiality.
